ORDER: Plaintiff's application to proceed in forma pauperis #4 is denied without prejudice. To continue with this case, Plaintiff must submit a renewed in forma pauperis application that is current and complete and that includes a copy of his trust fund statement showing activity in the account for the six- month period immediately before he filed this suit (August 2018-February 2019). Alternatively, he may prepay the $400 statutory filing fee. Plaintiff's failure to comply with this order by April 11, 2019 will result in summary dismissal of this case. His motion for an attorney #5 is denied without prejudice to him repeating his request if his case proceeds further. The Clerk of Court shall send Plaintiff a blank in forma pauperis application. Because Plaintiff complains of inadequate care for a medical condition, for which he may still be receiving some treatment, the Clerk is directed to send a copy of this order to Dixon Correctional Center Warden John Varga.
