ORDER signed by the Honorable Philip G. Reinhard on 4/8/2019: Plaintiff's application for leave to proceed in forma pauperis #4 is denied without prejudice. Plaintiff is directed to either (1) submit a properly completed application that is timely certified by a trust fund officer and that includes a copy of plaintiff's trust fund statements showing all transactions in his account or accounts for the entire six-month period immediately preceding the submission of his complaint; or (2) pay the full statutory filing fee of $400.00. Plaintiff is also ordered to show cause as to why he should be allowed to proceed in forma pauperis or why the court should not dismiss the case based on inaccuracies in the application submitted #4 . Failure to comply with this order by May 6, 2019, will result in summary dismissal of this case. Plaintiff's motion for attorney representation #6 is denied without prejudice as it is too early to determine if recruitment of counsel is warranted. Plaintiff's motion for service at government expense #5 is denied as unnecessary. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If plaintiff fails to keep the court informed of his address, this action will be subject to dismissal for failure to comply with a court order and for failure to prosecute. The Clerk of Court is directed to send plaintiff a blank application to proceed in forma pauperis and a copy of this order. [see STATEMENT] Mailed notice (kms)
