July 18, 2019 Opinion or Order Filing 7 ORDER: Plaintiffs request to proceed in forma pauperis (IFP) #2 is denied without prejudice, and his complaint #1 is dismissed without prejudice. To proceed IFP, Plaintiff must submit an application on this Courts form and include a certificate from an authorized officer at his facility and Plaintiffs trust fund account statement for the six-month period before he filed suit. See N.D. Ill. Local Rule 3.3 (all IFP applications in this Court must be on the Courts form); see also 28 U.S.C. 1915(a)(2). Alternatively, he may prepay the $400 filing fee. As Plaintiff should know from his prior cases, proceeding IFP does not waive payment of the filing fee, but simply allows him to pay with monthly deductions from his facilitys trust account. See 28 U.S.C. 1915(b)(2). Plaintiffs complaint must also be submitted on the Courts form, see Local Rule 81.1, and should clearly identify the parties sought to be named as Defendants, clearly state Plaintiffs claims against each Defendant, and provide information about his prior cases. If Plaintiff fails to submit an IFP and complaint consistent with this order by August 30, 2019, this case will be dismissed without prejudice.
