Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If plaintiff wants to proceed with this case, he must file an amended in forma pauperis application that accurately discloses his income beginning twelve months prior to initiating this action (in other words, October 23, 2018) and ending on the date of compliance. In addition, for this same time period, he must specify what he bought and explain why he believes his income should not be counted against his ability to prepay the full filing fee. Alternatively, plaintiff may prepay the $400 filing fee. In addition, plaintiff's complaint is dismissed with leave to submit an amended complaint using the court's form. If plaintiff fails to comply by January 8, 2020, the court will summarily dismiss this case. Plaintiff's motion for attorney representation #4 is denied without prejudice as he did not use the court's form. Plaintiff must promptly submit a change-of address notification if he is released or transferred to another facility. If he does not keep the Clerk of Court apprised of his correct address, this case will be subject to dismissal. Finally, a review of plaintiff's trust fund ledgers shows that in just the past six months, the trust fund custodian marked four gifts as "not held" and presumably did not deduct filing fee installment payments from this income. This is incorrect. The trust fund custodian is reminded of the obligation to deduct fees in accordance with the PLRA and the court's orders and should carefully review this order and ensure that proper deductions are made moving forward. The Clerk of Court is directed to send plaintiff an application for leave to proceed in forma pauperis, an amended complaint form, the motion for attorney representation form, and a copy of this order. The Clerk of Court is also directed to send a copy of this order to the trust fund custodian at the Dixon Correctional Center. Signed by the Honorable Philip G.
