The parties shall jointly complete the PDF fillable initial status report form found on Judge Jensen's webpage under Initial Status Conferences and Reports and file by 3/19/2020. The Clerk is directed to mail plaintiff a copy of the Court's initial status report form. Lead defense counsel with full knowledge of the case must appear in person at the initial status hearing. December 17, 2019 Opinion or Order Filing 7 ORDER: Plaintiff has filed a complaint #1 and a revised application for leave to proceed in forma pauperis ("IFP") #6 . Plaintiff's IFP application demonstrates that he cannot prepay the filing fee,and the application is thus granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $5.42 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. Additionally, the Court has conducted an initial review of Plaintiff's complaint and determined that it satisfies the requirements of 28 U.S.C. 1915. However, the Court dismisses Lieutenant Reicks and Lieutenant Murton. Accordingly, the Clerk of Court is directed to: (1) send a copy of this order to the trust fund officer of the facility having custody of Plaintiff and to the Court's Fiscal Department; (2) file Plaintiff's complaint; (3) issue summonses for service of the complaint on Defendants O'Sullivan, Albertson, the U.S. Attorney for the Northern District of Illinois, and the Attorney General of the United States; and (4) send Plaintiff four blank USM-285 (Marshals service) forms, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must complete and return a USM-285 form for service on each Defendant. Failure to return the USM-285 forms by January 6, 2020, may result in dismissal of any unserved Defendant, as well as dismissal of this case in its entirety. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
