ORDER : Plaintiff's application for leave to proceed in forma pauperis #4 is granted. The court orders the trust fund officer at plaintiff's place of incarceration to immediately deduct $36.70 from plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Clerk of Court is directed to send a copy of this order (electronically if possible) to the trust fund officer at the facility currently having custody of plaintiff and the court's fiscal department. Summonses shall not issue at this time, as the court dismisses plaintiff's complaint #1 for failure to state a claim and because it is not on the court's required form. Plaintiff is granted until December 2, 2020, to submit an amended complaint on the court's form that addresses the concerns in this order. If he fails to comply, the court will dismiss this case for failure to state a claim. Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not comply and fails to receive and respond to court orders as a result, this case will be subject to dismissal with no further warning. The Clerk of Court is directed to send plaintiff an amended civil rights complaint form, instructions, and a copy of this order. [See STATEMENT] Signed by the Honorable Philip G.
