Filing 7 ORDER : By order dated October 15, 2020, the court denied plaintiff's application for leave to proceed in forma pauperis without prejudice and ordered plaintiff to either submit a properly completed application, including supporting documents, or pay the court's $400.00 filing fee #5 . Plaintiff was also admonished that he must promptly submit a change-of-address notification if he is transferred to another facility or released. At that time, the court warned plaintiff that failure to comply with the court's order would result in summary dismissal of this case. Plaintiff has neither submitted a renewed application for leave to proceed in forma pauperis nor paid the court's filing fee to date. In addition, his mail was returned with the notation that plaintiff is no longer in custody. Accordingly, this case is dismissed without prejudice for plaintiff's failure to comply with the court's order and failure to prosecute his case. Filing 5 ORDER signed by the Honorable Philip G. Reinhard on 10/15/2020: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is certified by an appropriate official and accompanied by a copy of plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by November 16, 2020, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the court informed of his address may lead to dismissal of this action for failure to comply with a court order and for want of prosecution. Plaintiff's motion for attorney representation #4 is denied as premature. See Pickett v. Chicago Transit Auth., 930 F.3d 869, 871 (7th Cir. 2019) (suggesting that motion for attorney representation is premature if filed "so early in the case that it was impossible to tell whether [the plaintiff] could represent himself adequately").
