Filing 5 ORDER The court denies plaintiff's motion to compel Dixon Correctional Center to "provide an audit of Plaintiff [sic] account prior to transferring to JTC" #4 . Illinois prisoners routinely submit account statements from institutions at which they are no longer incarcerated. It is plaintiff's responsibility to gather and submit the statutorily mandated documents if he wants this court to consider an application to proceed in forma pauperis. See 28 U.S.C. 1915(a)(2). If the Joliet Treatment Center cannot provide the necessary account statements, plaintiff may need to submit a written request to Dixon to obtain a statement for the relevant time period. The court will grant plaintiff additional time to complete this task. If plaintiff wants to proceed further, he has until March 12, 2021, to either (1) file a properly completed and supported application to proceed in forma pauperis, or (2) prepay the $402 filing fee. Failure to comply will result in summary dismissal of this case. The Clerk of Court is directed to send plaintiff a copy of this order, a blank application to proceed in forma pauperis, and a copy of the January 7, 2021 order #3 setting forth the requirements for prisoners applying to proceed in forma pauperis.
