ORDER; On April 15, 2021, the Court ordered Plaintiff to pay the full $402 filing fee (or submit an application to proceed in forma pauperis) and submit an amended complaint on the Court's standard form by May 17, 2021. #4 Plaintiff has submitted an amended complaint #5 , but he has not paid the filing fee (or submitted an application to proceed in forma pauperis). Plaintiff's April 20, 2021 motion #6 , which asks the Court to order Dixon Correctional Center "to take filing fees off [his] trust fund account," is denied. Plaintiff is responsible for arranging for payment of the filing fee in this case (and he appears to have done so based on his representations in his later-filed motion [see 7]). Plaintiff's May 18, 2021 "motion to inform" #7 , which the Court construes as a request for an extension of time to pay the filing fee, is granted. Plaintiff is given until June 18, 2021 to pay the filing fee. If Plaintiff fails to comply by the aforementioned date, the Court will summarily dismiss this case. The Clerk is in instructed to send a copy of this order to the trust fund officer at Dixon Correctional Center and to Plaintiff. Signed by the Honorable Iain D. ORDER with Statement; Plaintiff has neither prepaid the filing fee nor sought leave to proceed in forma pauperis in this action. In addition, his handwritten complaint #1 is not on the Court's mandatory form (and does not otherwise provide information required by that form, such as Plaintiff's litigation history). If Plaintiff wants to pursue this litigation, by May 17, 2021, he must: (1) pay the full statutory filing fee of $402 or submit an application for leave to proceed in forma pauperis; and (2) submit an amended complaint on the Court's standard form. If Plaintiff does not comply with this order in its entirety by the aforementioned date, this case will be summarily dismissed. Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case will be subject to dismissal with no further warning. The Clerk is directed to send Plaintiff a blank application for leave to proceed in forma pauperis, an amended complaint form, instructions, and a copy of this order.
