ORDER: By order of May 24, 2021, the court directed Warden Ciolli to respond, by June 7, 2021, to plaintiff's allegation that Counselor Barmes refused to certify his trust fund application #7 . To date, no response has been received. The court directs the response to be filed by June 21, 2021. ORDER: Plaintiff has "struck out" under 28 U.S.C. 1915(g). He has requested leave to proceedunder the "imminent danger" danger exception, but the court defers any analysis as to that issue.Plaintiff's in forma pauperis application #5 is denied without prejudice as incomplete because it is neither certified by a trust fund officer nor does it include a copy of his trust fund statement for the six-month period preceding the filing of this complaint. To proceed with this lawsuit, plaintiff must submit a properly completed in forma pauperis application that: (1) accurately discloses his income and assets starting twelve months prior to initiating this action and ending on the date ofsubmission; and (2) includes a certified trust fund ledger that shows deposits into his accountbeginning six months prior to filing this lawsuit and through the date of submission. The courtrequests that the trust fund officer at USP Thomson grant plaintiff's reasonable requests forinformation necessary to complete the application. It is up to the prison to decide what policiesapply when so doing, and plaintiff must comply with those policies. Alternatively, plaintiff mayprepay the $402 filing fee by sending a check or money order (made payable to Clerk, UnitedStates District Court) to the Clerk, United States District Court, 219 South Dearborn Street,Chicago, Illinois 60604, attn: Cashier's Desk, 20th Floor. His payment should clearly identify hisname and the case number assigned to this case. Plaintiff's failure to fulfill the filing-feerequirements by June 28, 2021, will result in summary dismissal of this case. Warden Ciolli isdirected to respond, by June 7, 2021, to plaintiff's allegation that counselor Barmes refused tocertify his trust fund application. The Clerk is directed to send a copy of this order to plaintiff, thetrust fund officer at USP Thomson, Warden Ciolli, and to email a copy of this order to Marc Bides,mbides@bop.gov.
