ORDER; Plaintiff Heriberto Ortiz, a federal prisoner, has attempted to initiate this case by filing a document captioned as an "injunction to halt reprisals" #1 . Plaintiff's pleading concerns retaliatory conduct allegedly taken against him by staff at USP Thomson for filing a lawsuit in the Eastern District of Kentucky. [Id.] To proceed with this matter, Plaintiff must file a complaint using the Court's standard form that sets forth a claim for relief. In addition, Plaintiff must either: (1) pay the full $402 filing fee; or (2) submit an application for leave to proceed in forma pauperis that includes certified trust fund information for the full six month period immediately preceding the filing of this lawsuit (or, from about October 26, 2020 to April 26, 2021). If Plaintiff fails to fully comply with this order by June 18, 2021, the Court will summarily dismiss the case. Plaintiff is advised that he has a continuing obligation to keep the Court apprised of his current address, and failure to do so will result in dismissal of this case for failure to comply with a court order and for want of prosecution. The Clerk is directed to send Plaintiff a pro se complaint form, an application to proceed in forma pauperis, and a copy of this order. Plaintiff's motion for the production of documents #3 -- which includes a request for "appellate documents," a section 2241 form, and "a copy of court rules" -- is denied.
