ORDER with Statement; The Court denies Petitioner's motion for leave to file in forma pauperis #3 without prejudice to reconsideration should he renew his motion in compliance with this order. Petitioner is given until June 18, 2021, to either file an in forma pauperis application on the enclosed form with the information required by 28 U.S.C 1915(a)(2) or to pay the $5.00 filing fee. If Petitioner takes no action by that date, the Court may dismiss this action without prejudice. Petitioner's motion for attorney representation, #4 is denied without prejudice. The Clerk is instructed to: (1) send Petitioner a blank in forma pauperis application with instructions; (2) terminate Respondent USP Thomson from the docket; (3) add Petitioner's present custodian, Andrew Ciolli, Warden, USP Thomson, as Respondent; and, (4) alter the case caption to Downey v.
