ORDER; On June 23, 2021, Plaintiff was ordered to (1) submit a completed application for leave to proceed in forma pauperis that includes the required documentation and certification or (2) pay the full statutory filing fee of $402.00. In addition Plaintiff was ordered to submit an amended complaint. Plaintiff has not complied with the Court's order. However, Plaintiff has submitted "letters" indicating he is attempting to pay the filing. Plaintiff has not addressed the needed amended complaint. The Court extends the deadline for Plaintiff to fully comply with the Court's June 23, 2021 order to September 17, 2021. ORDER with Statement; This matter was recently transferred to this Court from the Middle District of Georgia. Plaintiff submitted a "complaint" #1 but did not prepay the filing fee or file an application for leave to proceed in forma pauperis. By July 30, 2021, Plaintiff must either: (1) submit a completed application for leave to proceed in forma pauperis that includes the documentation and certification described below; or (2) pay the full statutory filing fee of $402.00. In addition, the "complaint" is not on the Court's complaint form. If Plaintiff seeks to continue with this matter in this Court, he must also submit an amended complaint on the Court's amended complaint form by this same deadline. If Plaintiff did not seek to have his "complaint" filed as a lawsuit, he should submit a motion to voluntarily dismiss the matter pursuant to Fed. R. Civ. P. 41(a)(1)(A)(i). If Plaintiff does not comply with this Order, the Court will summarily dismiss this case. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis, an amended civil rights complaint form, and one blank USM-285 (Marshals Service) form.
