ORDER with Statement; Plaintiff's second application for leave to proceed in forma pauperis #6 is denied as incomplete. The Court also is concerned about the authenticity of the trust fund officer certification. By September 27, 2021, Plaintiff must: (1) prepay the $402 filing fee or submit a properly completed and supported application that includes trust fund account information for at least January 1, 2021, through the application date; and (2) submit a written explanation regarding the two certifications he submitted in this case, including the information requested below. Failure to comply with this order by the above date will result in dismissal of this case for fraud on the Court, along with other potential sanctions as explained below. The Clerk of Court shall send to Plaintiff, at his current place of incarceration, a copy of this order and of the certification he submitted in this case, page two of #6 . The Clerk of Court also shall send Plaintiff a copy of the certification from the first application he submitted, page two of #3 . See the attached order for details. Signed by the Honorable Iain D. ORDER with Statement; Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed and complete application that complies with the below instructions; or (2) pay the full filing fee of $402. If Plaintiff fails to comply with this order by August 27, 2021, the Court will summarily dismiss this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis for incarcerated individuals and a copy of this order. Plaintiff is warned that he must promptly submit a written change-of address notification if he is released or transferred to another facility. If he does not keep the court apprised of his correct address, this case may be dismissed for failure to follow a court order or failure to prosecute. See the attached order for details.
