ORDER with Statement; The Court dismisses Plaintiff's complaint #1 without prejudice for failure to state a claim. If Plaintiff wants to proceed further, he must submit an amended complaint on the Court's required form that states a federal claim stemming from the events alleged in the original complaint. Failure to submit an amended complaint by November 5, 2021, will result in dismissal of this lawsuit with prejudice for failure to state a claim. Plaintiff also is advised that he must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Clerk of Court is directed to send Plaintiff an amended complaint form and instructions along with a copy of this order. See the attached Order for details. Signed by the Honorable Iain D.
