Filing 5 ORDER signed by the Honorable Philip G. Reinhard on 1/4/2022: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. By February 11, 2022, plaintiff must submit either a properly completed application or a declaration, signed under penalty of perjury, explaining what steps he has taken to obtain a completed application, including when and with whom he discussed his requests. Alternatively, plaintiff may pre-pay the $402.00 filing fee. Additionally, plaintiff must submit a declaration, again under penalty of perjury, explaining why he contends that the "imminent danger" exception to 28 U.S.C. 1915(g) applies to his complaints of mistreatment and excessive force by officers that occurred months prior to the filing of this complaint. The Clerk is directed to send plaintiff a copy of this order and a blank in forma pauperis application. The Clerk also is directed to send a copy of this order to the trust fund officer at USP Thomson, who is requested to comply with plaintiff's reasonable requests for information to complete his in forma pauperis application, consistent with prison policies. Plaintiff's failure to fully comply with this order by the date set forth above will result in summary dismissal of this lawsuit.
