January 24, 2022 Opinion or Order Filing 6 ORDER : Plaintiff's application for leave to proceed in forma pauperis #5 is denied without prejudice. If Plaintiff wishes to proceed with this lawsuit, he must: (1) submit a renewed in forma pauperis application in accordance with this order; or (2) pay the full $402 filing fee. Plaintiff is given until February 25, 2022, to comply with this order. Failure to do so will result in summary dismissal of this lawsuit. The Clerk is instructed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. December 15, 2021 Opinion or Order Filing 4 ORDER; Plaintiff James Edward Johnson, an inmate at the Winnebago County Jail, initiated this pro se civil rights action without prepaying the filing fee or seeking leave to proceed in forma pauperis. Instead, he submitted a two-page "prisoner balance history report" #3 . If Plaintiff wants to pursue this litigation, he must pay the full statutory filing fee of $402 or submit an accurate and properly supported application for leave to proceed in forma pauperis. If Plaintiff fails to do so by January 24, 2022, this case will be summarily dismissed. Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility. Failure to do so will result in dismissal of this case for failure to comply with a Court order and for want of prosecution. The Clerk is directed to send Plaintiff a blank application for leave to proceed in forma pauperis and a copy of this order.
