Filing 5 ORDER; Plaintiff's application for leave to proceed in forma pauperis #4 is denied without prejudice. If Plaintiff wants to pursue this lawsuit, she must, by April 8, 2022, either (1) pre-pay the $402.00 filing fee, or (2) seek leave to pay the filing fee in installments by submitting a properly completed application for leave to proceed in forma pauperis. Plaintiff's motion to amend her complaint #3 is denied. If she wants to proceed with this case, Plaintiff must, in addition to satisfying the filing-fee requirements: (1) submit an amended complaint containing all claims and Defendants, and properly disclosing her litigation history; and (2) show cause (explain in writing) under penalty of perjury as to why she should not be sanctioned, up to and including dismissal of this case with prejudice, for failure to accurately disclose her litigation history in her original complaint. Failure to fully comply with this order by the date set forth above will result in summary dismissal of this case. The Clerk is directed to send Plaintiff a blank amended complaint form, a blank in forma pauperis application (prisoner version), and a copy of this order. See the attached Order for details.
