Filing 11 ORDER, Plaintiff has now paid the filing fee for this action. The Clerk of Court is directed to: (1) file Plaintiff's complaint #1 and issue summons for service of the complaint #1 on Defendant Kaluvik; (2) terminate Defendants the John Doe Doctor, the Jane Doe Nurse, the John Doe Nurse, Warden Wilks, Warden Tact, Director Jeffreys, and the State of Illinois; and (3) send Plaintiff one blank USM-285 service form, a magistrate judge consent form, filing instructions, and a copy of this order. Because Plaintiff is not proceeding in forma pauperis and has not demonstrated that she is unable to pay a service fee, she is not entitled to service at government expense. Fed. R. Civ. P. 4(c)(3). Therefore her motion for service of process at government expense #4 is denied. For the sake of efficiency, however, the Court appoints the USMS to serve Defendant. The USMS will contact Plaintiff directly for payment of the service fee. Plaintiff is encouraged to employ the USMS to effect service but may choose to retain a private process server. If Plaintiff decides to have the USMS serve Defendant, she must promptly pay the service fee by sending a check to the USMS (not the Clerk's Office), along with a completed USM-285 form for Defendant, so the USMS can timely effect service. Failure to complete service within 90 days of entry of this order may result in dismissal of this lawsuit. Fed. R. Civ. P. 4(m). Plaintiff must promptly update her address if it changes while this case is open. Failure to comply may subject this lawsuit to dismissal. Plaintiff's motion for attorney representation #5 is denied without prejudice. Plaintiff's motion for a status hearing #10 is denied as moot in light of this order. Filing 9 ORDER; Plaintiff's application for leave to proceed in forma pauperis #7 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application, using the Court's form, that is signed by him, accompanied by a copy of his trust fund statements showing activity in his account or accounts for the entire time he was incarcerated during the six-month period immediately preceding his initiation of this action, and certified by an appropriate official; or (2) pay the full statutory filing fee of $402.00. Failure to comply with this order by July 18, 2022, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the Court informed of his address may lead to dismissal of this action for failure to comply with a Court order and for want of prosecution. Plaintiff's motion for a copy of the complaint #8 is granted this is a one-time courtesy; in the future, Plaintiff should make copies of his Court filings.
