ORDER Plaintiff's application for leave to proceed in forma pauperis #9 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $4.36 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order (100% of Plaintiff's income - this includes gifts, prison payroll, settlements, etc. - when it meets statutory minimums, must be devoted toward his filing fees). The Court directs the Clerk of Court to (1) send a copy of this order to the trust fund officer at Dixon Correctional Center and to the Court's Fiscal Department; (2) file Plaintiff's complaint #1 ; (3) issue summonses for service of the complaint on the Defendants; and (4) send Plaintiff seven blank USM-285 (Marshals service) forms and a copy of this order. Plaintiff must complete and return a USM-285 form for service on each Defendant. Failure to return the USM-285 may result in dismissal of the unserved Defendant, as well as dismissal of this case in its entirety. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendants. Plaintiff's "motion and notice to the court" #7 is denied as the motion fails to seek any necessary relief from the Court (of course, Plaintiff cannot provide case numbers that he does not have). Plaintiff must refrain from peppering the Court with miscellaneous filings/motions. Plaintiff is an experienced litigation who knows the litigation process. ORDER, Plaintiff's application for leave to proceed in forma pauperis #5 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is certified by an appropriate official and accompanied by a copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action and include an accurate accounting of his income from all sources for the twelve month period for any period of time not incarcerated; or (2) pay the full statutory filing fee of $402.00. Failure to comply with this order by June 16, 2023, will result in summary dismissal of this case. The Clerk is directed to mail Plaintiff a blank application to proceed in forma pauperis application and a copy of this order. Plaintiff's "motion for relief" #4 is granted to the extent that Plaintiff need not submit copies of his filings when he submits filings to the Court. See the attached Order for details. ORDER, Joshua Hoskins, presently in state custody at Dixon Correctional Center, initiated this action under 42 U.S.C. 1983. Although Plaintiff submitted his complaint, he did not submit an application to proceed in forma pauperis that complies with the statutory requirements for prisoners, see 28 U.S.C. 1915(a)(2), nor did he pay the filing fee. If Plaintiff wants to proceed further, he has until June 15, 2023, to either (1) submit a completed application to proceed in forma pauperis on the enclosed form along with a certified copy of his trust fund account statement showing all transactions to his account for the six-month period immediately preceding his initiation of this action; or (2) pre-pay the $402.00 filing fee. Failure to comply with this order by this date will result in summary dismissal of this case. The Court defers initial review of Plaintiff's complaint #1 until his fee status is resolved.
