Filing 6 ORDER William H. Carr initiated this lawsuit under 42 U.S.C. 1983 while detained at the Winnebago County Jail, but he did not pay the Court's filing fee or apply for leave to proceed in forma pauperis. By order dated May 25, 2023, the Court ordered him to do one of two things: (1) prepay the $402.00 filing fee or (2) apply to proceed in forma pauperis using the court's form. The Court also explained that any application to proceed in forma pauperis must be accompanied by a certified account statement showing all transactions in his custodial account or accounts from November 18, 2022, to May 18, 2023. Plaintiff did not comply. The docket reflects, however, that he has changed address twice since the inception of this lawsuit #4 #5 . The Court therefore grants him additional time to resolve his fee status. By July 28, 2023, Plaintiff must comply with the instructions in the May 25, 2023 order #3 . If he fails to do so, this case will be summarily dismissed. Having initiated no fewer than five prior lawsuits in this court, Plaintiff is familiar with the procedures for initiating lawsuits here. The Court therefore will grant him no further extensions of time to resolve his fee status, absent a showing of good cause. The Court directs the Clerk of Court to send Plaintiff a copy of the May 25, 2023 order #3 along with a blank application to proceed in forma pauperis to his address of records. Filing 3 ORDER, Winnebago County detainee William H. Carr initiated this lawsuit under 42 U.S.C. 1983 concerning his medical care at the Winnebago County Jail, but he did not pay the Court's filing fee or apply for leave to proceed in forma pauperis. He must do one of two things: (1) prepay the $402.00 filing fee or (2) apply to proceed in forma pauperis using the enclosed form. An application to proceed in forma pauperis must be accompanied by a certified account statement showing all transactions in his custodial account or accounts from November 18, 2022, to May 18, 2023. If he was incarcerated at more than one facility during that time, he must obtain an account statement from each facility. Alternatively, he may submit payment of the $402.00 by check or money order payable to Clerk, United States District Court.
