ORDER: Plaintiff Matthew William Travis, an inmate at the McHenry County Jail, brings this pro secivil rights lawsuit, 42 U.S.C. 1983, alleging that jail officials failed to protect him from an attackby another inmate. Plaintiff initiated this lawsuit without pre-paying the $405.00 filing fee or filingan in forma pauperis application. If he wants to proceed with this case, Plaintiff must, by May 2,2026, either (1) pre-pay the $405.00 filing fee or (2) apply to proceed in forma pauperis using theenclosed form. An application to proceed in forma pauperis must be accompanied by a certifiedaccount statement showing all transactions in Plaintiff's trust fund account for the six monthspreceding the filing of this lawsuit from any institution at which he was confined during that period.28 U.S.C. 1915(a)(2). Payment may be sent to the Clerk of Court, United States District Court,219 South Dearborn Street, Chicago, Illinois 60604, attn: Cashier's Desk, 20th Floor, and mustclearly identify Plaintiff's name and the case number assigned to this case. Plaintiff should beaware that he must pay the filing fee regardless of the outcome of the case. See 28 U.S.C. 1915(b)(1). Thus, he must either pay the full filing fee upfront or pay the filing fee over time ininstallments consisting of 20% of his monthly income (including money sent by family and friends)until the filing fee is paid in full. If Plaintiff is allowed to pay in installments, the collections fromhis account will follow him even if he is transferred to another facility. Waiver of the fee is not anoption. Butler v. Deal, 794 F. App'x. 542, 544 (7th Cir. 2020) (unpublished). Plaintiff's failure tocomply with this order by the date set forth above will result in summary dismissal of this case.The Clerk is directed to send Plaintiff a copy of this order and a blank in forma pauperis application(prisoner version).
