The Clerk of the Court is DIRECTED to terminate these Defendants as parties in CM/ECF. IT IS FURTHER ORDERED that the Complaint (Counts 1 and 2) shall receive further review as to the Unidentified Defendants. The Clerk of the Court is DIRECTED to rename the Unknown Correctional Officers Defendant as follows: John Doe # 1 (C/O on duty 5/24/15 in N2-5 Gallery); John Doe # 2 (Crisis Team Member on duty 5/24/15 in N2-5 Gallery); John Doe # 3 (Lt. on duty 5/24/15 in N2-5); John Doe # 4 (Sgt. On duty 5/24/15 in N2-5 and N2-8 gallery); and John Doe # 5 (C/O on duty 5/24/15 in N2-8 Gallery). FURTHER, the Clerk of the Court is DIRECTED to add JACQUELINE LASHBROOK, the warden of Menard, in her official capacity, so that she may partic ipate in discovery aimed at identifying the Unknown Defendants with particularity. The Clerk of the Court shall prepare for Defendant LASHBROOK: (1) Form 5 (Notice of a Lawsuit and Request to Waive Service of a Summons), and (2) Form 6 (Waiver of Service of Summons).
