Defendants BROWN and WEXFORD HEALTH SOURCE, INC., are DISMISS ED from this action without prejudice. IT IS FURTHER ORDERED that if Plaintiff wishes to move the Court to reinstate the medical negligence claims in COUNT 2 and/or COUNT 5 against Defendants SCOTT and WEXFORD, respectively, Plaintiff shall file the required affidavit(s) pursuant to 735 Ill. Comp. Stat. §5/2-622, within 35 days of the date of this order (on or before September 12, 2017). With reference to COUNTS 1, 3, and 4, the Clerk of Court shall prepare for Defendants SCOTT, RECTOR, PIERCE, and CHAPMAN: (1) Form 5 (Notice of a Lawsuit and Request to Waive Service of a Summons), and (2) Form 6 (Waiver of Service of Summons).
