Order re: Doc. 1. Plaintiff's letter, which refers to alleged constitutional violations occurring at the Monroe County Jail, has been docketed as a Complaint. However, it does not meet several requirements per the Federal Rules of Civil Procedure. For example, Plaintiff has not included a case caption listing defendants per Fed. R. Civ. P. 10, or a proper request for relief per Fed. R. Civ. P. 8. In order to assist Plaintiff, the Clerk of Court is DIRECTED to send Plaintiff a form complaint. Plaintiff shall submit a proper complaint or, in the alternative, indicate that he does not wish to proceed with this action within 28 days of this order, on or before December 27, 2018.
