Under 28 U.S.C. 1915, an indigent party may commence a federal court action without paying required costs and fees upon submission of an affidavit asserting the inability to pay such fees or give security therefor and stating, the nature of the action, defense or appeal and the affiants belief that the person is entitled to redress. 28 U.S.C. 1915(a)(1). The document submitted by Plaintiff is insufficient to satisfy 28 U.S.C. 1915 and does not contain the information required for this Court to make the necessary determination. Accordingly, The Clerks Office is DIRECTED to mail an in forma pauperis application and financial affidavit to Plaintiff. Nonetheless, in order to protect the identity of Plaintiff's minor child, the Court DIRECTS the Clerk of the Court to redact the personal data identifiers pertaining to Plaintiff's minor child.
