For the reasons stated in the attached Memorandum & Order, Plaintiff's motion for leave to proceed in forma pauperis is GRANTED. This matter also is before the Court for purposes of docket control. Berry continues to ignore the Court's directive to return a signed consent/non-consent form in a timely manner. Cognizant of Berry's status as a pro se litigant, the Court DIRECTS the Clerk of Court to send another copy of the form to Berry along with a copy of this Order.
