Plaintiff should also complete a summons form for Deputy Brad within this 30-day p eriod. The Clerk of Court is DIRECTED to send Plaintiff a copy of page one and two of a 42 U.S.C. § 1983 complaint form with this case number and the word "amended" written in the caption along with one summons form to Plaintiff. cc: Plaintiff (pro se) w/ forms, Defendants, LaRue County Attorney (JM)
