The Clerk of Court is DIRECTED to send Plaintiff a prisoner 1983 complaint form with this case number and AMENDED affixed thereto for Plaintiffs use should he wish to filean amended complaint. Plaintiff is WARNED that should he fail to file an amended complaint within 30 days, his freedom of religion claim will be dismissed for the reasons set forth herein.
