ORDER by Senior Judge Joseph H. McKinley, Jr on 2/21/24: Plaintiff's motion for a docket sheet [DN #6 ] is DENIED as moot. Accordingly, IT IS ORDERED that no later than 3/25/2024, Plaintiff must file his complaint on the proper form and attach the necessary summonses. The Clerk of Court is DIRECTED to send Plaintiff a 1983 Prisoner Packet. IT IS FURTHER ORDERED that no later than 3/25/2024, Plaintiff must either pay the $405.00 filing fee or file an application to proceed without prepayment of fees and affidavit form along with a certified copy of his prison trust account statement for the six month period preceding the filing of the petition. Should Plaintiff wish to pay the $405.00 filing fee, a check shall be made payable to Clerk, U.S. District Court and mailed to the following address: Office of the Clerk, United States District Court, Western District of Kentucky, 601 West Broadway, Ste. 106, Louisville, KY 40202-2249. Should Plaintiff seek to proceed in forma pauperis in this action, the Clerk of Court is DIRECTED to send Plaintiff an application to proceed without prepayment of fees for his completion. Plaintiff is WARNED that his failure to comply with this Order will result in dismissal of the action for failure to prosecute and for failure to comply with an Order of this Court. cc: Plaintiff(pro se w/1983 Prisoner Packet w/IFP Motion) (DJT)
