October 27, 2023 DOCKET ANNOTATION as to # 4 and # 5. Any PDF fillable on-line forms once completed should be electronically converted to PDF by printing to the PDF printer. Save the file with an appropriate name in a folder. E-file the electronically converted PDF form, not the form which was filled in and saved. Clerk's Office has printed both forms and scanned them in to replace what was filed incorrectly. The judge initials were incorrect on document # 5 and the plaintiff was cut off in the style on both applications. Clerk's Office has corrected all errors. In the future, attorney should print all fillable forms to pdf, verify judge initials for accuracy, and be sure that the plaintiff's name is complete and correct in the style of the case. Attorney does not need to re-file. (JCH)
