Filing 2 ORDER that the Clerk of the Court SHALL SEND Plaintiff the approved form application to proceed in forma pauperis by a prisoner, as well as the document entitled information and instructions for filing an in forma pauperis application. FURTHER ORDER ED that within thirty (30) days from the date of this order, Plaintiff shall either: file a fully complete application to proceed in forma pauperis or pay the full $400 filing fee. FURTHER ORDERED that Plaintiff shall submit a complaint to this Court within thirty (30) days from the date of this order. FURTHER ORDERED that the Clerk of the Court shall send to Plaintiff the approved form for filing a 42 U.S.C. § 1983 complaint and instructions for the same. The Clerk of the Court shall also send Plaintiff a copy of his petition for maturity claim/counterclaim and maturity of counterclaim. Signed by Magistrate Judge Nancy J. Koppe on 6/20/16.
