If the Court declines to med iate this case, the answer/response shall be due thirty (30) days following the order declining mediation. The parties SHALL DETACH, COMPLETE, AND FILE the attached Notice of Intent to Proceed with Mediation form on or before thirty (30) days from t he date of the entry of this order.
