Filing 5 MINUTE ORDER IN CHAMBERS of the Honorable Magistrate Judge Valerie P. Cooke, on 4/23/2012. By Deputy Clerk: Lisa Mann. Plaintiffs application to proceedin forma pauperis 1 . is GRANTED. However, plaintiff must pay the entire filing fee even if this action is dismissed. The NDOC shall pay to the Clerk of the USDC, 20% of the preceding months deposits to plaintiffs account (inmate #64984), in the months that the account exceeds $10.00, until the full $350.00 filing fee has been pa id for this action. The Clerk of the Court shall SEND a copy of this order to the Finance Division of the Clerks Office (Sent). The Clerk shall also SEND a copy of this order to the attention of the Chief of Inmate Services for the Nevada Department of Corrections (Sent). The Clerk shall DETACH and FILE the complaint (ack'd). IT IS FURTHER ORDERED that the clerk of the court shall ISSUE summons to Michael Astrue, US Atty Genreal, and the USA (ack'd). The clerk shall deliver the summo ns to the U.S. Marshal for service. The clerk shall send plaintiff sufficient copies of the complaint and service of process forms (USM-285) for each defendant.Plaintiff shall have twenty (20) days in which to return to the required forms USM-285 to the U.S. Marshal.
