Filing 21 MEMORANDUM AND ORDER: The Court grants Plaintiff's 4 request to proceed in forma pauperis pursuant to 28 U.S.C. § 1915 solely for the purpose of this Memorandum and Order. Accordingly, the Court dismisses the Complaint for failure to state a claim. The Court grants Plaintiff thirty (30) days from the date of this Memorandum and Order to file an amended complaint. To aid Plaintiff with this task, the Court respectfully requests the Clerk of Court to provide Plainti ff with a "Complaint for Violation of Civil Rights" form and an in forma pauperis form. If Plaintiff fails to timely file an amended complaint, the Court will enter judgment dismissing this action for the reasons set forth above. The Court certifies pursuant to 28 U.S.C. § 1915(a)(3) that any appeal from this Memorandum and Order would not be taken in good faith, and therefore in forma pauperis status is denied for the purpose of an appeal.
