July 7, 2020 Opinion or Order ORDER: On May 12, 2020, Plaintiff Lesimeon Brooks, proceeding pro se, commenced action by alleging that he contracted COVID-19 in custody and that the jail failed to provide him with adequate medical care. (ECF No. 1.) Plaintiff was then-detained at Rikers Island on a parole violation. (Id.) On May 14, 2020, the Clerk of Court alerted Plaintiff that in order to satisfy filing requirements and to proceed in forma pauperis ("IFP") with his action, he needed to submit additional information in his IFP application. (ECF No. 2.) The Clerk enclosed a copy of the in forma pauperis form and directed plaintiff to complete and return it within 14 days. (Id.) The letter expressly warned: "If you do not comply, your case will not proceed and may be dismissed." (Id.) According to the New York City Department of Corrections "Inmate Lookup Service," Plaintiff was released from Rikers Island on May 26, 2020. See http://a073-ils-web.nyc.gov/inmatelookup/pages/common/find.jsf (accessed June 15, 2020). To date, Plaintiff has not returned the form or otherwise responded to the Clerks letter; nor has he provided the Court with a current mailing address, as is his obligation. Plaintiff shall return a completed copy of the in forma pauperis form within 30 days. Failure to do so will result in dismissal of this case. The Clerk of Court is respectfully directed to mail a copy of the in forma pauperis form and this order to the pro se litigant. Ordered by Judge LaShann DeArcy Hall on 7/7/2020. (Williams, Erica)
