ORDER - Accordingly, plaintiff is ORDERED to complete and return the enclosed PLRA within 14 days from the date of this Order so that this case may proceed. Plaintiff is warned that his failure to timely comply may lead to the dismissal of his complaint without prejudice for failure to prosecute pursuant to Rule 41 of the Federal Rules of Civil Procedure. The Court certifies pursuant to 28 U.S.C. 1915(a)(3) that any appeal from this Order would not be taken in good faith and therefore in forma pauperis status is denied for the purpose of any appeal.
