For the foregoing reasons, Plaintiffs' motion is GRAN TED in its entirety. The Clerk of Court is respectfully requested to close the motion pending at docket number 63, and to amend the case caption to (1) terminate Raul Ortega and Araceli Ortgea; (2) add Manuel Medina as a Defendant; and (3) add Silviana Casarez as a Plaintiff. Filing 37 OPINION AND ORDER. For the reasons in this Opinion and Order, Plaintiffs' motion for conditional certification of a collective action is granted. Plaintiffs are directed to submit a revised proposed notice and opt-in form consistent with this Or der within two weeks. Within ten days of the Court's approval of the final notice and opt-in form, Defendants shall post copies at the Restaurant in a location conspicuous to all employees. Defendants are further directed to provide to Plaintiff s, within twenty days of the date of this Order, an electronic file containing the names, last known mailing addresses, last known email addresses, and last known telephone numbers of all waitresses and dancers who worked at the Restaurant between Ju ly 23, 2010 and the present. The parties shall confer and reach agreement on a Spanish translation of the final notice and opt-in form. Finally, Plaintiffs are directed to submit a letter within thirty days of this Order advising the Court as to whet her they intend to move for class certification.
