September 21, 2018 Opinion or Order Filing 7 VALENTIN ORDER: To allow Plaintiff to effect service on Dr. Thah, counsel for H+H shall, within thirty days of the date of this order, determine whether Dr. Thah is or was an H+H or PAGNY employee, and provide an address where he may be served, either at his residence or via PAGNY. See, e.g., Portillo v. Hassan, No. 17-CV-6675 (VSB) (S.D.N.Y.) (directing H+H counsel to provide addresses where defendants could be served). Once counsel has done so, the Court will, if necessary, issue an order directing the Clerk of Court to complete the USM-285 forms with the address for service on Defendant Thah and deliver to the U.S. Marshals Service all documents necessary to effect service. The Court directs the Clerk of Court to mail a copy of this order and the complaint to counsel for New York City Health + Hospitals at: 125 Worth Street, New York, New York 10013. The Clerk of Court is also directed to mail a copy of this order to Plaintiff, together with an information package. The Court certifies under 28 U.S.C. 1915(a)(3) that any appeal from this order would not be taken in good faith, and therefore in forma pauperis status is denied for the purpose of an appeal. Cf. September 11, 2018 Opinion or Order Filing 4 ORDER GRANTING IFP APPLICATION IN PRISONER CASE: The Clerk of Court is directed to send a copy of this order and the prisoner authorization to the agency having custody of Plaintiff. That agency is directed to forward copies of Plaintiff's prison trust fund account for the past six months and to disburse the payments required under 28 U.S.C. 1915 to the United States District Court for the Southern District of New York and to include the above docket number on the disbursement before sending it to the Court. If Plaintiff is transferred to another facility, the current facility shall provide a copy of this order to the facility to which Plaintiff is transferred.
