October 29, 2018 Opinion or Order Filing 7 ORDER OF SERVICE: terminating #4 Application for the Court to Request Counsel. The Clerk of Court is directed to (i) terminate the motion (Doc. # 4) and (ii) mail a copy of this order to Plaintiff, together with an information package. The Clerk of Court is further instructed to issue a summons and deliver all necessary documents to Plaintiff for service on Defendants J. Lamanna, Superintendent of Green Haven Correctional Facility; B. Kelly, Deputy Superintendent of Administration; and Russo, Deputy Superintendent of Security. Plaintiff is directed to serve Defendants J. Lamanna, Superintendent of Green Haven Correctional Facility; B. Kelly, Deputy Superintendent of Administration; and Russo, Deputy Superintendent of Security within 90 days of the issuance of the summons. Plaintiff must notify the Court in writing if his address changes, and the Court may dismiss the action if Plaintiff fails to do so. Local Civil Rule 33.2 applies to this action. The Court certifies under 28 U.S.C. 1915(a)(3) that any appeal from this order would not be taken in good faith, and therefore in form a pauper is status is denied for the purpose of an appeal. Cf. Coppedge v. United States, 369 U.S. 438, 444-45 (1962) (holding that an appellant demonstrates good faith when he seeks review of a nonfrivolous issue). SO ORDERED. (Signed by Judge Vincent L.
