Before a case can be assigned to a magistrate judge for all proceedings, including a final order and a trial, all parties must sign and date Form AO85. Because Defendants have not signed and dated Form AO 85, the Court denies without prejudice Plaintiff's request to reassign the case to a magistrate judge. Plaintiff may renew his request after Defendants appear in this action. ORDER OF SERVICE: Plaintiff, who is proceeding pro se, paid the filing fees to commence this action. The Clerk of Court is directed to issue summonses as to Defendants Dr. Neil C. Evans, Shanay Webster, Carol Johnson, Dr. Mark J. Mulligan, and George Reed Grimes. Plaintiff is directed to serve the summons and complaint on each Defendant within 90 days of the issuance of the summonses.
