| Minute Entry for proceedings held before Judge Cathy Seibel: Pre-Motion Conference held on 1/17/2024. Plaintiff Linda Remy appears telephonically. Defendant Chromalloy Corporation counsel Robert Weissman, Esq. appears telephonically. Law Clerk Tatiana Codner appears telephonically. The Court hereby directs that the Plaintiff sign up to receive electronic notification(s) from the Court. Defendant's counsel directed to send Plaintiff a copy of Defendant's December 15, 2023 letter (Doc # 5) again by mail and email. The Court grants Plaintiff leave to amend Complaint. ORDER DIRECTING ADDRESS AND ORIGINAL SIGNATURE: Plaintiff is directed to submit, within 30 days of the date of this order (1) the signed signature page of her complaint, and (2) a written notice of her mailing address or a consent to accept electronic service. A copy of the signature page and a consent form to accept electronic service are attached to this order. The Court directs the Clerk to mail this order to Plaintiff at the following address: Jordan Place, Hillcrest, NY 10977. If Plaintiff fails to comply with this order within the time allowed, the action will be dismissed without prejudice. The Court certifies under 28 U.S.C. 1915(a)(3) that any appeal from this order would not be taken in good faith, and therefore in forma pauperis status is denied for the purpose of an appeal. Cf. Coppedge v. United States, 369 U.S. 438, 444-45 (1962) (holding that appellant demonstrates good faith when seeking review of a nonfrivolous issue). SO ORDERED.
