ORDER. Plaintiff's renewed application for leave to proceed in forma pauperis, Docket Item 7, is denied without prejudice. The Clerk of Court shall administratively terminate this action without filing the Complaint or assessing a filing fee. The Court will grant Plaintiff a final opportunity to fix his errors, and Plaintiff has until September 25, 2021 to either (1) submit a signed Affidavit and Certification or (2) pay the $402.00 in filing fees. If Plaintiff fails to comply with this deadline, this case will be dismissed without prejudice. To assist Plaintiff, the Clerk of Court shall send Plaintiff a blank in forma pauperis application form that includes the Certification.1 Upon submission of the Affidavit and Certification or the filing fee by September 25, 2021, the Clerk of Court shall reopen this case without further order. This writing must include either (1) a properly supported motion to proceed in forma pauperis along with the required certification of Plaintiff's inmate trust fund account (or the institutional equivalent) and authorization form or (2) the $350.00 filing fee and the $52.00 administrative fee ($402.00 total); and it is further ORDERED that upon Plaintiff's submission of either (1) a motion to proceed in forma pauperis along with the required certification of Plaintiff's inmate trust fund account and authorization form or (2) the $350.00 filing fee and the $52.00 administrative fee ($402.00 total), the Clerk of Court shall reopen this case without further order; and it is further ORDERED that Plaintiff's Motion for Miscellaneous Relief (Docket Item 3) is denied as moot.
