ORDER denying #2 Application for Leave to Proceed in forma pauperis. For these reasons, the application (Doc. No. 2) is DENIED without prejudice to Plaintiff's ability to submit a second, properly supported application. The Clerk of Court is DIRECTED to mail Plaintiff a new blank application to proceed in forma pauperis and a copy of Administrative Order 93. Accordingly, at the same time that he submits his IFP application, Plaintiff MUST file an amended complaint in which he clearly identifies his claims, describes the particular actions and/or inactions by each Defendant about which he complains and provides the location and approximate date of each such event. The Clerk is DIRECTED to mail Plaintiff a blank Section 1983 complaint form for Plaintiff's use in complying with this Order.
