ORDER: Plaintiff's IFP application does not satisfy that requirement. Accordingly, Plaintiff's pending application #2 is DENIED for failure to attach the required trust account statement, without prejudice to Plaintiff's ability to submit a second, properly supported application. The Clerk of Court is DIRECTED to mail Plaintiff a blank IFP application (short form) and a copy of Administrative Order 93. To proceed with his lawsuit, Plaintiff MUST either pay a $400 filing fee in full1 or complete the application and return it to the Court with the required certified trust account statement. Plaintiff's payment or supported application must be received in this Court within 30 days of the date this Order is entered on the docket and must include the docket number assigned to this case: No. 1:20-cv-0058. Plaintiff is cautioned that if he does not comply with this Order within the time frame specified, or if he fails to promptly notify the Court of any change in his address, this action may be dismissed for failure to prosecute and for failure to comply with the Court's Order.
