Filing 5 ORDER: This case cannot proceed without further action by the Plaintiff. First, Plaintiff has not signed the complaint. (Doc. No. #1 at 12). Every pleading must be signed by a party personally if the party is unrepresented by counsel. See Fed. R. Civ. P. 11(a). Therefore, the Clerk is DIRECTED to return the original complaint to Plaintiff so that she can comply with Rule 11 (a). Plaintiff, in turn, is DIRECTED to return the signed complaint within 30 days of the date this Order is entered. Second, in order to file a civil lawsuit, a plaintiff must submit the civil filing fee or an application to proceed in forma pauperis. In addition, if seeking pauper status, a prisoner-plaintiff must provide a certified copy of her inmate trust fund account statement for the 6-month period immediately preceding the filing of his complaint as required by 28 U.S.C. 1915(a)(2). Plaintiff submitted an application to proceed in forma pauperis, AO 240 (Short Form). (Doc. No. #2 ). However, in her complaint, Plaintiff states that she was released from jail on November 20, 2020, and now resides at 392 Rabbit Trail Lane, Leona, Tennessee 38468. (Doc. No. #1 at 13). The Court requires nonprisoners to submit AO Form 239 (Long Form) when seeking pauper status. Therefore, the Clerk is DIRECTED to mail Plaintiff AO Form 239 for her convenience. Plaintiff, in turn, is DIRECTED to return the completed application within 30 days of the date this Order is entered if she is no longer incarcerated and she wishes to proceed in forma pauperis. Third, Plaintiff provides two addresses in her submissions to the Court: (1) the residential address in Leona, Tennessee and (2) Debra K. Johnson Rehabilitation Center, 3881 Stewarts Lane, Nashville, Tennessee 37218. It is Plaintiff's responsibility to keep the apprised of her current mailing address. Out of an abundance of caution, the Clerk is DIRECTED to mail this Order to Plaintiff at both addresses provided in her submissions. However, Plaintiff SHALL clarify to which address she wishes her mail to the be sent and whether she has been released from custody. Signed by District Judge William L. Campbell, Jr on 12/14/2020.
