The facility custodian of Plaintiff's inmate account should simply sign (using his or her full name) and date the inmate trust fund statement, which makes the statement "certified." For Plaintiff's convenience, the Clerk is DIRECTED to mail Plaintiff a copy of Doc. No. #5 so Plaintiff does not incur additional copying costs. Also within 30 days, Plaintiff must complete and return his Application for Leave to Proceed in District Court Without Prepaying Fees or Costs for prisoners (Short Form, AO 240) if he wishes to seek pauper status. he wishes to seek pauper status.
