Filing 4 ORDER: Bernard Woodard, an inmate of the Bledsoe County Correctional Complex in Pikeville, Tennessee, filed this pro se action under 42 U.S.C. 1983 against Eddie Farris, Phil Arms, Tim Nash, f/n/u Brandon, and Dillard Shift. (Doc. No. #1 ). In order to file a civil lawsuit, a plaintiff must submit the civil filing fee or an application to proceed in forma pauperis. The Clerk is DIRECTED to send Plaintiff a blank application to proceed in forma pauperis for prisoners. Plaintiff, in turn, is DIRECTED to do one of the following within 28 days of the date he receives this Order: either return to the district court a properly completed application to proceed in forma pauperis; or Plaintiff may submit the full civil filing fee to the Clerk of Court.
