ORDER: The application (Doc. No. #1 ) is DENIED without prejudice to Plaintiff's ability to submit a second, properly supported application. The Clerk of Court is DIRECTED to mail Plaintiff a new blank application to proceed in forma pauperis and a copy of Administrative Order 93. If Plaintiff seeks to have this Court consider his complaint, he MUST either pay the $400.00 filing fee in full or complete the IFP application and return the properly completed application, along with an updated trust account statement, to the Court. Plaintiff's submissions must be received in this court within 28 days of the date this Order is entered on the docket and must include the docket number assigned to this case, No. 3:19-cv-00664. At the same time, Plaintiff must file an amended complaint in which he clearly identifies the location and approximate dates of the alleged violations of their civil rights and describe the particular acts by each Defendant about which they complain.
