Filing 4 ORDER: Timothy Eugene Kelly, an inmate of the DeBerry Special Needs Facility in Nashville, Tennessee, has filed a pro se civil rights complaint. (Doc. No. #1 .) Included within the complaint is the plaintiff's "Request to be filed Under Indigent" (id. at 4), which the court construes as an application for leave to proceed in forma pauperis (IFP). However, the plaintiff's IFP application is incomplete. Accordingly, the Clerk of Court is DIRECTED to mail the plaintiff a blank IFP application form. If the plaintiff seeks to have this court consider his complaint, he MUST either pay the $400.00 filing fee in full or complete the IFP application form and return it to the court.
